Case Manager - care planning (Boston, MA)

Clearpath Thistle Early Learning is looking for a Case Manager. The job centres on care planning for classroom supplies, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Boston. • Applications are reviewed on a rolling basis.
