Case Manager - care planning (Boston, MA)

Amberfield Haven Early Learning is an early-learning provider in Boston. We are hiring a Case Manager to take on care planning. Low staff turnover is a goal we take seriously. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Boston. • Applications are reviewed on a rolling basis.
