Case Manager - care planning (Boston, MA)

We are growing our team at Ironwood Sparrow Health and adding a Case Manager whose main responsibility will be care planning. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Boston. • We reply to every applicant.
