Case Manager - care planning (Boston, MA)

Bluepeak Sparrow Care Services Group supports families and individuals in Boston with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 42 people based in Boston. What you will do • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boston. • Applications are reviewed on a rolling basis.
