Case Manager - care planning (Boise, ID)

Nova Meridian Health is looking for a Case Manager. The job centres on care planning for referrals, and it would suit someone who likes steady, practical work. You would join a small organisation of about 39 people based in Boise. What you will do • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boise, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
