Case Manager - care planning (Boise, ID)

We are growing our team at Ironwood Ridge Learning Group and adding a Case Manager whose main responsibility will be care planning. Families are partners. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Boise. • We reply to every applicant.
