Case Manager - care planning (Boise, ID)

Clearpath Granite Community Partners is a nonprofit in Boise supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Boise, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
