Case Manager - care planning (Birmingham, AL)

Rivermark Quill Health is a healthcare provider in Birmingham with clinics and support teams. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 30 people based in Birmingham. What you will do • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Birmingham, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
