Case Manager - care planning (Birmingham, AL)

Redstone Amber Community Partners runs community programmes across Birmingham. We are hiring a Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Birmingham. • Applications are reviewed on a rolling basis.
