Case Manager - care planning (Birmingham, AL)

We are growing our team at Clearpath Thistle Learning Group and adding a Case Manager whose main responsibility will be care planning. Families are partners. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Birmingham. • We will tell you what to expect at each step of the hiring process.
