Case Manager - care planning (Birmingham, AL)

We are growing our team at Cedar Maple Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 47 people based in Birmingham. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Birmingham. • We will tell you what to expect at each step of the hiring process.
