Case Manager - care planning (Birmingham, AL)

Ironwood Kestrel Community Partners runs community programmes across Birmingham, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Birmingham. • We reply to every applicant.
