Case Manager - care planning (Birmingham, AL)

We are growing our team at Clearpath Beacon Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Birmingham. What you will do • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Birmingham, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
