Case Manager - care planning (Birmingham, AL)

Clearpath Basin Civic Services Group works with county and city offices around Birmingham, and we need a Case Manager to lead our care planning. You would join a small organisation of about 31 people based in Birmingham. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for public records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Full-time, with a steady weekday schedule. • On site in Birmingham. • We will tell you what to expect at each step of the hiring process.
