Case Manager - care planning (Birmingham, AL)

Greenridge Haven Property Management looks after rental homes across the Birmingham area, and we need a Case Manager to lead our care planning. Residents are neighbours, not numbers. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Birmingham. • Applications are reviewed on a rolling basis.
