Case Manager - care planning (Baltimore, MD)

We are growing our team at Redstone Lantern Learning Group Associates and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 36 people based in Baltimore. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Baltimore. • Applications are reviewed on a rolling basis.
