Case Manager - care planning (Baltimore, MD)

Westbrook Nest Housing Group manages affordable housing communities across Baltimore. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 11 people based in Baltimore. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Baltimore. • We reply to every applicant.
