Case Manager - care planning (Austin, TX)

Fernwood Linden Health is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records.
