Case Manager - care planning (Anchorage, AK)

We are growing our team at Rivermark Vale Property Management Partners and adding a Case Manager whose main responsibility will be care planning. Maintenance and leasing staff share information every day. What you will do • Help with applications and appointments for unit turnovers. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Anchorage. • Applications are reviewed on a rolling basis.
