Case Manager - care planning (Anchorage, AK)

Bright Meridian Health is a healthcare provider in Anchorage with clinics and support teams, and we need a Case Manager to lead our care planning. You would join a small organisation of about 19 people based in Anchorage. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Anchorage, with some flexibility after the first few months. • We reply to every applicant.
