Case Manager - care planning (Anchorage, AK)

Amberfield Yarrow Civic Services works with county and city offices around Anchorage, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Anchorage. • We reply to every applicant.
