Case Manager - care planning (Anchorage, AK)

Summit Meridian Care Services provides in-home and community support in Anchorage. We are hiring a Case Manager to take on care planning. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Anchorage. • We will tell you what to expect at each step of the hiring process.
