Nova Cross Health Partners is looking for a Case Manager. The job centres on care planning for patient scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 198 people based in Anchorage. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Anchorage. • Applications are reviewed on a rolling basis.
