Case Manager - care planning (Anchorage, AK)

Lumen Haven Learning Group is looking for a Case Manager. The job centres on care planning for school calendars, and it would suit someone who likes steady, practical work. You would join a large organisation of about 435 people based in Anchorage. What you will do • Help with applications and appointments for school calendars. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish.
