Fernwood Mill Care Services is looking for a Case Manager. The job centres on care planning for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Albuquerque. • Applications are reviewed on a rolling basis.
