Case Manager - care planning (Albuquerque, NM)

We are growing our team at Nova River Property Management Group and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Albuquerque. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent collection. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Albuquerque. • Applications are reviewed on a rolling basis.
