Case Manager - benefits navigation (Toledo, OH)

Fernwood Meadow Community Partners Group runs community programmes across Toledo, and we need a Case Manager to lead our benefits navigation. We are small enough to know everyone's name. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, on a regular day shift. • On site in Toledo. • We will tell you what to expect at each step of the hiring process.
