Case Manager - benefits navigation (Toledo, OH)

Amberfield Meadow Housing Group manages affordable housing communities across Toledo, and we need a Case Manager to lead our benefits navigation. Collaboration with local agencies is constant. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Toledo. • We reply to every applicant.
