Case Manager - benefits navigation (Toledo, OH)

We are growing our team at Stonegate Labs and adding a Case Manager whose main responsibility will be benefits navigation. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Toledo, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
