Case Manager - benefits navigation (St. Louis, MO)

We are growing our team at Amberfield Mill Care Services Group and adding a Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 49 people based in St. Louis. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in St. Louis. • We will tell you what to expect at each step of the hiring process.
