Case Manager - benefits navigation (Spokane, WA)

We are growing our team at Summit Union Health Group and adding a Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Spokane. • Applications are reviewed on a rolling basis.
