Lumen Fern Care Services Group is looking for a Case Manager. The job centres on benefits navigation for visit scheduling, and it would suit someone who likes steady, practical work. You would join a small organisation of about 32 people based in Spokane. What you will do • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling.
