Case Manager - benefits navigation (San Jose, CA)

Westbrook Nest Housing Group manages affordable housing communities across San Jose. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 24 people based in San Jose. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident services. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
