Case Manager - benefits navigation (San Jose, CA)

We are growing our team at Lumen Juniper Health Group and adding a Case Manager whose main responsibility will be benefits navigation. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
