Case Manager - benefits navigation (San Jose, CA)

Greenridge Quill Health is a healthcare provider in San Jose with clinics and support teams, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in San Jose. • We reply to every applicant.
