Case Manager - benefits navigation (San Jose, CA)

We are growing our team at Northwind Sparrow Care Services Group and adding a Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in San Jose. • We reply to every applicant.
