Case Manager - benefits navigation (San Jose, CA)

Fernwood Collective provides primary and specialty care in San Jose. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 8 people based in San Jose. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
