Case Manager - benefits navigation (San Diego, CA)

Cedar Basin Health Group is a healthcare provider in San Diego with clinics and support teams, and we need a Case Manager to lead our benefits navigation. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • On site in San Diego, with some flexibility after the first few months. • We reply to every applicant.
