Case Manager - benefits navigation (San Antonio, TX)

Greenridge Hollow Care Services is a social-care provider serving older adults and people with disabilities around San Antonio. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in San Antonio. • We reply to every applicant.
