Cedar Glen Health Group is looking for a Case Manager. The job centres on benefits navigation for patient scheduling, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling.
