Case Manager - benefits navigation (Salt Lake City, UT)

Ironwood West Care Services Partners supports families and individuals in Salt Lake City with personal care and coordination, and we need a Case Manager to lead our benefits navigation. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • On site in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
