Bright Ivory Health Partners is looking for a Case Manager. The job centres on benefits navigation for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Salt Lake City. • Applications are reviewed on a rolling basis.
