Case Manager - benefits navigation (Sacramento, CA)

Highbridge Wren Health is a healthcare provider in Sacramento with clinics and support teams, and we need a Case Manager to lead our benefits navigation. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • On site in Sacramento, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
