Case Manager - benefits navigation (Sacramento, CA)

Ironwood Oak Health is a healthcare provider in Sacramento with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Sacramento. • Applications are reviewed on a rolling basis.
