Case Manager - benefits navigation (Sacramento, CA)

Westbrook Lakeside Care Services Group is a social-care provider serving older adults and people with disabilities around Sacramento, and we need a Case Manager to lead our benefits navigation. We train and promote from within. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Sacramento. • We will tell you what to expect at each step of the hiring process.
