Case Manager - benefits navigation (Sacramento, CA)

We are growing our team at Bluepeak Mill Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 13 people based in Sacramento. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Sacramento. • We reply to every applicant.
