Case Manager - benefits navigation (Sacramento, CA)

Summit Thistle Learning Group operates schools and learning programmes around Sacramento, and we need a Case Manager to lead our benefits navigation. Families are partners. What you will do • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Sacramento. • Applications are reviewed on a rolling basis.
