Case Manager - benefits navigation (Richmond, VA)

Highbridge Ivory Health provides primary and specialty care in Richmond. We are hiring a Case Manager to take on benefits navigation. You would join a large organisation of more than 1,000 people based in Richmond. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Richmond, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
