Case Manager - benefits navigation (Reno, NV)

Highbridge Basin Health Group provides primary and specialty care in Reno. We are hiring a Case Manager to take on benefits navigation. Continuing education is supported. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Reno, with some flexibility after the first few months. • We reply to every applicant.
