Case Manager - benefits navigation (Reno, NV)

We are growing our team at Northwind Glen Health Partners and adding a Case Manager whose main responsibility will be benefits navigation. Continuing education is supported. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Full-time, with a steady weekday schedule. • On site in Reno. • Applications are reviewed on a rolling basis.
